Listing Questionnaire: Care & Services

About 10 minutes. Reply to our email (or write listings@senior.living.community) with your community name and the numbered answers — skip anything that doesn’t apply. Partial answers are welcome.

Care levels

  1. Which care levels do you offer? (independent living, assisted living, memory care, skilled nursing, respite/short stays)
  2. How do care-level assessments work at your community, and how often are residents reassessed?
  3. Can a resident age in place as needs grow — and where is the line (what needs would require a move)?
  4. Can couples live together when they need different levels of care?

Medical support

  1. Medication management: reminders only, or full administration?
  2. Nursing coverage (24/7 RN, daytime RN, LPN, on call)
  3. Therapy services available on site (PT / OT / speech) — in-house or visiting?
  4. Visiting practitioners (physician, podiatry, dentistry, mental health)?
  5. Do you coordinate with hospice for end-of-life care on site?
  6. Conditions you routinely support (diabetes/insulin, oxygen, catheter, two-person transfers) — and any you cannot

Non-medical services

  1. What’s included in the base monthly rate (housekeeping frequency, laundry, linens, utilities, cable/internet)?
  2. What’s billed separately, and roughly how is it priced (care points/levels, à la carte)?
  3. Personal care help available (bathing, dressing, escorts to meals/appointments)

Who can live here

  1. Age requirement (55+, 62+, none) and any exceptions (younger spouse, adult disabled child)
  2. Pet policy: allowed? size/type limits, deposits or fees, and what support exists if a resident can no longer care for the pet
  3. Do you accept Medicaid (directly or after a private-pay period)? Long-term care insurance?
