Listing is free for our founding cohort of advisors. We verify everything below before publishing — that’s the point: a listing here tells families you can document what the best in this industry already do. Email your answers (and attachments) to advisors@senior.living.community.

Business basics

  1. Legal business name, DBA, state(s) of registration, and year founded (we verify with the Secretary of State)
  2. States and metro areas you actively serve, and your office address in each
  3. Are you independent or part of a franchise/network? Which?
  4. Approximate placements completed in the last 12 months, and the settings you place into (assisted living, memory care, small care homes, independent living, skilled nursing)

Compliance & credentials

  1. Oregon: your Long-Term Care Referral Registry number · Nevada: state registration status · Washington: confirm RCW 18.330 compliance and attach your disclosure statement template
  2. Attach your certificate of liability insurance (carrier, limits, expiration)
  3. Have you and all client-facing staff passed a criminal background check in the last 3 years? Through whom?
  4. Credentials held (CPRS, CSA, RN/LSW, other) with certificate numbers so we can verify with the issuer
  5. Memberships (NPRA and chapter, OSRAA, ASRP-WA, other) — may we verify with the organization?

Business model & conflicts

  1. Who pays you, and how is your fee computed? Do families ever pay? Attach the exact written disclosure a family receives and tell us when they receive it
  2. Roughly what share of communities in your market pay you referral fees? Will you recommend a community that doesn’t pay you if it’s the best fit? Give a real example
  3. Any compensation from communities besides placement fees (marketing fees, bonuses, gifts)? Any ownership interest in a community you refer to?
  4. Your refund/fee policy if a placement fails within 30/60/90 days

Service model

  1. Describe your intake assessment (care needs, budget, Medicaid trajectory). Do you serve families who will need Medicaid within 24 months?
  2. Do you tour communities with families in person? What share of last year’s placements included an accompanied tour?
  3. When did you last personally visit the communities you most often recommend, and how do you check inspection histories before recommending?
  4. What does your follow-up after move-in look like, and for how long?

References & history

  1. Two family references from placements in the past 18 months, and one professional reference (social worker, discharge planner, or a community that does not pay you)
  2. Any state attorney general complaints, lawsuits, registration denials/revocations, or BBB complaints in the past 5 years? Explain
  3. Links to your review profiles — we look at review age and spread, not just the star average

We treat references and documents confidentially; what we publish is your listing profile and your verbatim answers to the conflict-of-interest questions. We may decline or delist for verified ethics violations — that protection is what makes the listing worth having.
